You are eligible to join the O.C. 500 Club as a Member if you meet the following criteria:

* Print this webpage and fill it out,
* Provide a league sheet from a USBC-certified league OR provide a recap sheet from a USBC-certified tournament
   OR have your League Secretary fill out and sign the League Secretary section below - in order to verify your 500 series, 
* With a copy of your USBC Card and appropriate fees shown below,
* Present the above items to Secretary/Treasurer Phyllis Dodge before a Club tournament (see Tournament Schedule).

* NOTE: All Members must independently purchase their own white polo or white collared button-down shirt to wear during the O.C. 500 Club tournaments (unless otherwise specified).  The O.C. 500 Club will provide shirt embroidery of your name and the Club logo for a small fee.
